This clinical trial is studying a new tool called the Readiness for EXtubation score (REXs) to help doctors decide when patients on a breathing machine (mechanical ventilation) in the intensive care unit (ICU) are ready to have the breathing tube safely removed. When patients rely on machines to help them breathe, doctors need to carefully check if they are stable enough to breathe on their own again. This study aims to create and test a score that looks at several health factors together to better predict if a patient can successfully come off the breathing machine without needing it again within two days.
Patients who are currently in the process of being weaned off a breathing machine in the ICU may be eligible to join, as long as they have a legal guardian to give consent, are not pregnant, do not have a tracheostomy (a different type of breathing tube), are not part of other studies, and do not have certain medical devices like diaphragmatic pacers. Participants can expect doctors to use this new scoring tool alongside their usual care to help decide the best timing for removing the breathing tube. This study could help improve care for seriously ill patients by making extubation decisions safer and more accurate.

Eligibility criteria
- Inclusion Criteria:
- • - Patients who are in the weaning process from mechanical ventilation after being connected to invasive mechanical ventilation in the intensive care unit.
- Exclusion Criteria:
- • Patients without legal guardian consent.
- • Tracheostomized patients.
- • Patients enrolled in other studies.
- • Individuals with diaphragmatic pacers.
- • Pregnant patients.
