This clinical trial is studying ways to help people with heart failure avoid going back to the hospital within 30 and 90 days after they leave. Heart failure is a common and serious condition in Ghana, and many patients end up being readmitted to the hospital, which can increase the risk of serious health problems or death. This study at Komfo Anokye Teaching Hospital is testing whether giving patients extra support, including detailed advice before they leave the hospital and regular phone calls afterward, can reduce the chances of needing to come back to the hospital soon after their first stay.
Adults aged 18 and older who have been diagnosed with moderate to severe heart failure (meaning they have symptoms that affect their daily activities) may be eligible to take part, as long as they have access to a mobile phone. Participants in the study will receive special counseling when they are discharged and follow-up calls to check on their health and answer any questions. This extra care aims to help patients manage their condition better and stay healthier at home.
