Comparison of Injection Techniques in Carpal Tunnel Syndrome

This clinical trial is looking at two different types of injections to treat carpal tunnel syndrome, a condition that causes pain, numbness, and weakness in the hand and wrist. The study will compare an ultrasound-guided injection using a sugar water solution (called 5% dextrose) with a corticosteroid injection, which is a type of medicine that reduces inflammation. The goal is to see which treatment works better in relieving symptoms and how long the benefits last.
To join the study, participants need to have moderate carpal tunnel syndrome in one hand, confirmed by a nerve test within the last six months. They should have symptoms like numbness, tingling, or weakness that have lasted more than a month and cause pain rated at least 4 out of 10. People who have had wrist surgery, certain nerve problems, infections, or previous steroid injections for carpal tunnel will not be eligible. If you join, you can expect to receive one of these injections and be monitored to see how your symptoms improve over time. This study is not yet recruiting, but it is open to adults of all genders who fit the criteria.

Eligibility criteria
- Inclusion Criteria:
- • Patients diagnosed with unilateral moderate carpal tunnel syndrome (CTS) confirmed by an electrophysiological study within the last 6 months
- * Presence of at least one of the following symptoms lasting more than one month:
- • Paresthesia/dysesthesia accompanied by weakness and clumsiness in the hand, worsened by repetitive wrist use or sleep, and relieved by postural correction or shaking of the hand
- • Numbness due to sensory disturbance in the median nerve (MN) distribution
- • Thenar muscle weakness with atrophy
- • Positive Tinel's sign or Phalen's test
- • Pain intensity of ≥4 out of 10 on the Visual Analog Scale (VAS) prior to treatment
- Exclusion Criteria:
- • History of previous wrist surgery, polyneuropathy, brachial plexopathy, or thoracic outlet syndrome
- • Presence of systemic infection
- • Pregnancy
- • Prior corticosteroid injection for CTS
