This clinical trial is comparing two different types of anesthesia used during weight-loss surgery (bariatric surgery) to see if avoiding opioids (strong pain-relief medicines) during the operation is safe and effective. Researchers want to find out if opioid-free anesthesia works just as well as the usual opioid-based approach in patients having surgery to help with obesity.
Adults aged 18 to 65 years who are scheduled for planned bariatric surgery and have a certain health status (classified as ASA I to III, meaning they are generally healthy or have mild to moderate health issues) may be eligible to join. Before surgery, participants will be randomly placed into one of two groups: one receiving anesthesia with opioids, and the other without opioids. All patients will get standard care before and during surgery, including medicines to reduce stomach acid and prevent nausea, as well as careful monitoring to keep anesthesia at the right level. If you qualify and agree to join, you will be informed clearly about the study and asked to give written consent at least a day before your surgery. This study aims to improve anesthesia care for people undergoing weight-loss surgery, and your participation could help doctors find safer options for managing pain during these operations.

- Inclusion Criteria:

- • BMI \>35
- • ASA class III-IV
- • Elective bariatric surgery (e.g., sleeve gastrectomy)
- • Written informed consent
- Exclusion Criteria:
- • Refusal to participate
- • Severe cardiac/pulmonary/psychiatric illness
- • Allergy to study drugs
- • Conversion to emergency surgery
- • Withdrawal at any point by patient or investigator
