ClinConnect Summary

This clinical trial is studying a new way to manage pain during and after gynecological surgeries in women who are severely overweight. Instead of using traditional opioid pain medicines, which can have side effects like confusion or addiction, this study will test a combination of a medication called dexmedetomidine and a special type of nerve block called a Transversus Abdominis Plane (TAP) block, guided by ultrasound. The goal is to see if this opioid-free approach can help women recover better, experience less pain, and have fewer problems like confusion after surgery.

Women between 21 and 65 years old who have a body mass index (BMI) of 35 or higher, are classified as having a moderate risk for anesthesia, and are scheduled for planned gynecological surgery may be eligible to participate. However, women with certain health issues like allergies to local anesthetics, liver or kidney problems, heart conditions, cognitive difficulties, or those taking long-term pain medications would not qualify. If chosen, participants will receive the opioid-free anesthesia during their surgery and be closely monitored to understand how well this method works for pain control and recovery. This study is not yet open for enrollment but aims to offer safer pain management options for women undergoing these surgeries.

Eligibility criteria

  • Inclusion Criteria:
  • 50 female patients aged 21-65 years.
  • Patients with a Body mass index≥ 35 kg/m2
  • American society of Anesthesiology class II-III.
  • Undergoing elective gynecological surgeries.
  • Exclusion Criteria:
  • Patient refusal.
  • History of allergy to local anesthetic.
  • Liver or renal pathology affecting drug elimination.
  • Mental dysfunction or cognitive disorders.
  • Patients on chronic pain medications.
  • Hypotension, bradycardia, atrioventricular block, intraventricular or sinoatrial block.
  • Coagulopathy
