This clinical trial, called the Discovery Diabetes study, is looking at a new program designed to help teenagers and young adults with type 1 diabetes better manage their condition. The study wants to find out if this program can improve blood sugar control (measured by a test called HbA1c), reduce feelings of stress related to diabetes, and help participants develop stronger self-care skills. The program includes personalized support through mobile phone messages and help from healthcare providers over three months. Researchers will compare people in the program to those receiving usual diabetes care to see if the program makes a positive difference.
To join the study, participants need to be between 13 and 21 years old, have had type 1 diabetes for at least one year, have access to a smartphone, and be able to use the My Health at Vanderbilt patient portal. They also need to speak and read English and have an appointment with a nurse practitioner at the Vanderbilt Eskind Clinic within seven months of joining. During the study, participants will complete surveys at the start, 3 months, and 6 months, and allow the team to review their medical records related to diabetes. The study is not yet recruiting, but it offers a chance for young people with type 1 diabetes to try new ways of managing their health with extra support.

Eligibility criteria
- Inclusion Criteria:
- • Adolescents (13-17 years) and young adults (aged 18-21 years),
- • Diagnosed with T1D for at least one year,
- • Access to a smartphone,
- • A My Health at Vanderbilt (MHAV) patient portal account,
- • Ability to read, speak, and understand English,
- • An appointment with an NP in the VUMC Eskind Clinic within 7 months of initial contact
- Exclusion Criteria:
- • Diagnosis of type 2 diabetes
- • Any physical, cognitive, sensory or emotional condition precluding participation in the intervention (seeing/using a phone, hearing digital stories, answering questions).
