This clinical trial is studying whether a single, focused conversation called a solution-focused consultation (SSC) can help people with both long-term physical health problems and mental health concerns, like anxiety or depression, to better engage with mental health services. The goal is to see if this one-time session can encourage people to get the support they need for their mental well-being.
People who might be eligible to join are adults who have one or more chronic physical illnesses, such as diabetes or heart disease, and who show signs of depression or anxiety based on simple questionnaires. They should not already be receiving formal mental health care and must be able to speak English. Participants will be receiving treatment at community healthcare centers, and anyone with a history of severe mental conditions like psychosis or intellectual disabilities would not be eligible. If you join, you can expect to take part in one focused session aimed at helping you identify practical steps to improve your mental health treatment engagement. This study is currently looking for volunteers to participate.

Eligibility criteria
- • Inclusion Criteria: 1. A history of one or more chronic physical diseases based on the Ministry of Health (Singapore) Chronic Disease Management Programme guidelines; 2. A score of 5 or more on the Patient Health Questionnaire (PHQ-9) or Generalized Anxiety Disorder-7 (GAD-7); 3. Not currently engaged in formal mental health service; 4. Conversant in the English language; 5. Receiving treatment in a community healthcare setting
- • Exclusion Criteria: 1. A history of psychosis, intellectual disability, or pervasive developmental disorders
