This clinical trial is studying two different ways to attach lower dentures to dental implants for people who have lost all their lower teeth. Everyone in the study will receive three implants in their lower jaw, and then they will get dentures that connect to these implants. The study compares a traditional bar attachment to a bar with extra small attachments called OT Equators, which may help keep the denture more secure and reduce wear over time. The researchers want to see which method holds the denture better, protects the bone around the implants, lasts longer, and makes patients more satisfied.
To join the study, participants need to be healthy adults between 35 and 65 years old who have no lower teeth and enough bone in their lower jaw to support the implants. People who smoke, have certain jaw problems, or have health conditions that affect healing won’t be eligible. If you take part, you will be monitored over several months with tests that measure how well your denture stays in place, how your bone around the implants is doing, and how worn the attachments become. You’ll also be asked about how happy you are with your denture. The study is happening at Cairo University, is free to join, and participation is completely voluntary.

Eligibility criteria
- Inclusion Criteria:
- • Completely edentulous healthy patients.
- • Age between 35 and 65 years.
- • Sufficient bone in the anterior and premolar-molar regions (≥11 mm).
- • Adequate inter-arch space (12-14 mm).
- Exclusion Criteria:
- • Severe maxillomandibular skeletal discrepancy.
- • Presence of parafunctional habits such as clenching or bruxism.
- • Temporomandibular joint disorders.
- • Smokers.
- • History of drug abuse.
- • History of head and neck radiation.
- • Systemic disorders that could prevent surgery or affect bone quality (e.g., uncontrolled diabetes, osteoporosis).
