This clinical trial is studying different ways to manage pain and improve knee movement after total knee replacement surgery. The goal is to see which type of regional anesthesia (a way to numb certain parts of the body) helps patients move their knee better and feel more satisfied with their recovery in the first few days after surgery. Ninety patients having knee replacement surgery will be divided into three groups, each receiving a different pain control method alongside spinal anesthesia. The study will measure how well patients can bend and straighten their knee on days 1, 2, and 3 after surgery, as well as their overall satisfaction, pain levels, and use of opioid pain medicines.

To take part, patients need to be adults scheduled for their first knee replacement, be generally healthy enough for spinal anesthesia, and agree to this type of pain control. People with certain health problems, infections, allergies to specific medicines, or who cannot cooperate with the study won’t be eligible. If you join, you can expect careful monitoring of your knee’s movement and pain after surgery, with the aim to find the best pain relief method to help you recover comfortably and quickly. The study is happening at Konya City Hospital and will begin recruiting soon.

Eligibility criteria

  • Inclusion Criteria:
  • Scheduled for primary total knee arthroplasty
  • Age ≥ 18 years
  • ASA physical status I-III
  • Accepting neuraxial anesthesia
  • Exclusion Criteria:
  • Skin infection at the planned injection site
  • Sepsis
  • Coagulopathy
  • Refusal of neuraxial anesthesia
  • Severe hypovolemia
  • Demyelinating central nervous system disease
  • Age under 18 years
  • Illiterate in Turkish
  • Non-cooperative patients
  • Known allergy or hypersensitivity to bupivacaine
