This clinical trial is studying whether doing pelvic floor physical therapy at home after surgery for endometriosis can help improve quality of life and reduce symptoms better than the usual care patients receive after surgery. Endometriosis is a condition where tissue similar to the lining inside the uterus grows outside it, often causing pain. The surgery in this study removes these growths without taking out the uterus.

Women who might join this study are those who are premenopausal, speak English, have chronic pelvic pain lasting at least six months, and are having endometriosis surgery at a specific hospital with specialized surgeons. They also need to have a certain type of pelvic floor muscle tightness found during an exam, and be able to communicate by phone or email. Participants will either follow a home physical therapy program focused on pelvic floor muscles after surgery or receive the usual care. The study will then compare how each group recovers and feels after the surgery. It’s important to know that this study is not yet enrolling patients and has some specific rules, such as excluding women who are pregnant, using opioids long-term, or need extra time to rest after surgery.

Eligibility criteria

  • Inclusion Criteria:
  • Premenopausal
  • English speaking
  • Undergoing endometriosis excision surgery without hysterectomy at Legacy Health with subspecialty-trained surgeons specializing in endometriosis care
  • History of chronic pelvic pain (defined as \>= 6 months of pain in the anatomic pelvis)
  • Diagnosis of high tone pelvic floor dysfunction on examiniation
  • Have access to reliable email and phone for study-related communication
  • Exclusion Criteria:
  • Non-English speaking
  • Unable to consent
  • Unable to sustain 30 minutes of moderate activity at baseline
  • Uses a mobility assistive device at baseline
  • Chronic opioid use
  • No histologic diagnosis of endometriosis on surgical pathology
  • Need for additional pelvic rest (\>2 weeks) postoperatively
  • Pregnancy or planning to pursue pregnancy or undergo fertility treatments within 12 weeks postoperatively
