ClinConnect Summary

This clinical trial is studying the best way to give blood transfusions to patients who are experiencing severe bleeding and shock, which can happen after an injury or serious stomach bleeding. The goal is to find the safest, fastest, and most effective method to help patients survive and recover. The study compares three different blood transfusion methods to see which works best in emergency situations.

Patients who might take part are between 10 and 90 years old and have severe bleeding causing shock, with low blood levels or ongoing blood loss, and who arrive at the hospital within 24 hours of their symptoms starting. People with very serious other health problems or a history of bad reactions to blood transfusions would not be eligible. If you join, you will be randomly assigned to receive one of three transfusion methods, and doctors will closely monitor your survival, how quickly your blood pressure improves, how much blood is used, and any side effects. This research aims to develop a clear, team-based approach to emergency blood transfusions that could help save more lives in the future.

Eligibility criteria

  • Inclusion Criteria:
  • Patients with hemorrhagic shock due to trauma or upper gastrointestinal bleeding who meet emergency transfusion criteria (hemoglobin \< 7 g/dL or active bleeding).
  • Age 10-90 years. Time from onset to hospital admission \< 24 hours.
  • Exclusion Criteria:
  • Severe underlying diseases (end-stage organ failure or active malignancy). Known coagulopathy or history of severe transfusion reactions. Refusal to participate or inability to complete follow-up.
