This trial is testing whether a digital self-help program for Applied Relaxation (AR), delivered through a web app called Relappy, can reduce jaw pain and improve daily functioning in people with Temporomandibular Disorders (TMD) more than the usual night-time occlusal splint (a mouthguard-like device). Participants are randomly assigned to either the AR program or the splint treatment and are followed for up to one year. Researchers will measure TMD pain and how much pain gets in the way of daily life, plus several related factors such as body pain, mood (depression and anxiety), sleep quality, and even some signs of autonomic nervous system activity in a subset of participants.
Who can participate and what to expect: adults or children with a TMD-related pain diagnosis can be eligible, as long as they have a full dentition (not missing teeth that would disrupt the bite) and do not have serious illnesses or pregnancy. Eligible participants will complete web-based questionnaires at baseline, then at about 6 weeks, 3 months, and 1 year, and those in the sleep study will complete a digital sleep diary. In Oulu, a smaller group will also have physiological tests like heart-rate and blood pressure measures. The study is being conducted in Finland and is currently enrolling; results are not yet available, with final completion expected around 2030.

Eligibility criteria
- Inclusion Criteria:
- • presence of TMD pain diagnosis (myalgia, myofascial pain with referral, arthralgia and/or headache attributed to TMD), based on validated, international Diagnostic Criteria for TMD Axis I criteria
- • the patient has a full dentition (lack of a single tooth in a balanced occlusion does not preclude participation)
- Exclusion Criteria:
- • serious general illnesses such as rheumatism, schizophrenia or other serious mental illness
- • pregnancy
