This clinical trial is studying different types of nerve blocks—special injections that help control pain—in older adults who have broken their hip. The main goal is to find out which nerve block works best to reduce pain before surgery, helps patients get into position more comfortably for spinal anesthesia, lowers the need for other pain medicines, and lasts long enough to help with recovery without slowing down physical therapy. Researchers will also look closely at any medical problems that might happen during the hospital stay after these nerve blocks, spinal anesthesia, and surgery.
The trial is open to men and women aged 60 and older who have a hip fracture caused by a simple fall or similar injury and are scheduled for surgery. It does not include people with multiple serious injuries, severe memory problems, certain kidney issues, or allergies to some common medicines used during the procedure. Participants will be carefully examined before surgery and closely watched afterward until they leave the hospital. Those in the study will either receive one of the nerve blocks or a usual pain medicine called fentanyl before surgery, allowing doctors to compare which approach works best and is safest.

Eligibility criteria
- Inclusion Criteria:
- • both sexes
- • 60 years old and older
- • diagnosed with a hip fracture resulting from low-energy trauma
- • scheduled surgery.
- Exclusion Criteria:
- • polytraumatized patients
- • dementia and acute psychosis patients
- • patients for whom spinal anesthesia is contraindicated
- • patients with severe renal failure
- • patients for whom intravenous administration of tranexamic acid is contraindicated
- • patients with proven allergy and contraindications to dexamethasone, non-steroidal anti-rheumatic drugs, opioid analgesics, magnesium sulfate.
