Development and Validation of a Comprehensive Management Service Package for Older Adults With Multimorbidity

This clinical trial is studying a new care program designed to help older adults who have two or more long-term health conditions, such as high blood pressure, diabetes, heart disease, arthritis, or memory problems. The program includes digital tools, personalized advice, and ongoing support to help manage these multiple health issues better than usual care. The goal is to see if this new approach can reduce hospital visits, improve overall quality of life, and help people carry out their daily activities more easily.
People who are 40 years or older and have at least two chronic health conditions may be eligible to join. If chosen, participants will be randomly assigned to either receive this new care program or continue with their usual medical care. The study will follow them for six months to track their health, how well they stick to treatments, and any safety concerns. This trial is not yet recruiting participants but aims to include about 394 people to better understand how to support older adults living with multiple health challenges.
