This clinical trial is studying a new way to use ventilators for patients with acute brain injuries, such as strokes or traumatic brain injuries. When patients have lung injuries, they often need help breathing with a machine called a ventilator. However, if the settings on the ventilator are too high, it can actually harm the lungs further. This study aims to implement a computerized protocol that uses lower settings on the ventilator while still ensuring that the patient’s carbon dioxide levels remain normal, which is important for recovery.
To be eligible for this study, patients must be at least 18 years old and have an acute brain injury due to causes like strokes or bleeding in the brain. They must also begin using a ventilator in the emergency department or intensive care unit of an Intermountain Healthcare hospital. Participants can expect that their care will follow a new, standardized approach to mechanical ventilation, which may help improve their overall recovery. This study is currently active but not recruiting new participants.

Eligibility criteria
- Inclusion Criteria:
- • 1. Acute brain injury due to non-traumatic causes (stroke, spontaneous intracranial hemorrhage, cerebral edema, anoxic brain injury) or traumatic brain injury.
- • 2. Initiation of mechanical ventilation in the emergency department or intensive care unit at an Intermountain Healthcare hospital
- • 3. Age ≥ 18 years
- Exclusion Criteria:
- • 1. Transition to comfort care in the emergency department or on the same day of admission to the ICU
- • 2. Death on the same day of admission to the emergency department or ICU
