This clinical trial is studying two different ways to help patients with heart failure after they leave the hospital. The goal is to see which method better reduces the chances of needing to go back to the hospital and improves patients' quality of life. One group will receive a follow-up phone call from a care coordinator to check on their recovery. The other group will have access to a community paramedic who can visit them at home if they need medical help. This paramedic can provide care right away or arrange a video appointment with a doctor.
To participate in this study, you must be between 65 and 74 years old, have heart failure, and be receiving care at NewYork Presbyterian or Mount Sinai Health Systems in New York City. You also need to be enrolled in Medicare or Medicaid. Unfortunately, those who speak languages other than English, Spanish, Mandarin, or French, or those with certain conditions like dementia or who are in hospice care, are not eligible. If you join the study, you can expect to receive either the phone check-in or the home visit from a paramedic, depending on which group you are assigned to. This trial is currently looking for volunteers to help improve care for heart failure patients.

Eligibility criteria
- Inclusion Criteria:
- • Medicare or Medicaid recipient
- • Current diagnosis of HF
- • Receiving inpatient care at NewYork Presbyterian or Mount Sinai Health Systems
- • Live in NYC
- Exclusion Criteria:
- • Non-English, Spanish, Mandarin, or French speaking
- • Diagnosis of dementia or psychosis
- • Anticipated discharge to, or current residence in, skilled nursing facility or rehab center
- • Anticipated discharge to, or currently receiving, hospice including home hospice
- • Current candidate for and awaiting heart transplant
- • Current left ventricular assist device (LVAD)
