The LOMBAFAST trial is studying a new approach to help people suffering from subacute and chronic low back pain, which is pain that lasts for more than six weeks. The goal of the trial is to see if a short program that combines education and rehabilitation can effectively improve patients' conditions and reduce the risk of them developing severe disability related to their back pain. Participants will also receive personalized follow-up care to support their recovery. This trial is open to men and women between the ages of 65 to 74 who have low back pain that hasn't improved with other treatments and who experience some fears about their pain.
To be eligible, participants should have experienced low back pain for at least six weeks, have been working without taking more than 30 days off in the past year due to pain, and be open to participating in rehabilitation exercises. If you or someone you know is interested in joining the trial, you should be prepared to answer some questions about your fears related to movement and recovery. This study is currently recruiting participants and aims to not only improve pain management but also assess the cost-effectiveness of this new program.

Eligibility criteria
- Inclusion Criteria:
- • Patients with subacute or chronic nonspecific low back pain (current episode duration ≥ 6 weeks), resistant to outpatient management, and including patients with radiculopathy as long as low back pain is predominant over pain in the lower extremity.
- • Patients with less than 30 days off work (declarative) for low back pain in the previous year and involved in a professional activity at the time of inclusion.
- * Patients manifesting fears and false beliefs and/or dramatization: to be included, the patient will have to answer yes to one of the following two questions:
- • Are you afraid to move?
- • Are you afraid you will never get out of it?
- • Patients eligible for a rehabilitation exercise program
- • informed and written consent to participate
- • Exclusion Criteria
- • Age \< 18 years,
- • Insufficient French language proficiency.
- • Patients having followed a multidisciplinary rehabilitation program for their low back pain in the 3 months preceding inclusion.
- • Patients who have had lumbar spine surgery within the last 12 months.
- • Pregnancy in progress.
- • No affiliation with a health insurance program.
