This clinical trial is studying a technique called Physiological Intracytoplasmic Sperm Injection (PICSI) to see if it can help reduce miscarriage rates in couples undergoing fertility treatment. PICSI works by selecting healthier, more mature sperm using a special dish that attracts only the best sperm. The goal is to find out if using PICSI can lead to fewer miscarriages after infertility treatments compared to the standard method known as Intracytoplasmic Sperm Injection (ICSI).
Eligible participants for this trial are couples receiving ICSI treatment at the Shropshire and Mid-Wales Fertility Centre who agree to use PICSI. Both partners must provide consent for their sperm and eggs to be used in this study. Participants will be randomly placed in one of two groups: one will receive standard ICSI treatment, while the other will have the added benefit of the PICSI technique to select sperm. This trial is currently recruiting participants, and it aims to gather more information on how effective PICSI might be in reducing miscarriage rates before moving on to a larger study.

Eligibility criteria
- Inclusion Criteria:
- • Patients undergoing ICSI as part of their treatment at the Shropshire and Mid-Wales Fertility Centre
- • A total of 140 couples coming through for treatment who have consented to the use of
- • PICSI in their treatment and participation in the PICSI study
- • Both parties providing gametes for use in treatment must give consent to participate in the study

- • • If testicular or frozen sperm samples are needing to be used
