This clinical trial is investigating a new surgical technique called Shaeer's Sub-Gluteal Internal Pudendal Vein Ligation (SHAEER-I) to help men who have erectile dysfunction caused by deep system veno-occlusive issues. The main goals of the study are to see if this surgery can improve the firmness of erections, how satisfied patients are after the procedure, and whether there are any complications.
To be eligible for this trial, men must have been diagnosed with deep system veno-occlusive erectile dysfunction and have not found relief from other treatments, such as medications or hormone therapy. However, not everyone can participate. Men with uncontrolled diabetes, certain other health issues, or those who refuse to join the study will not be included. Participants who decide to take part will either undergo the new surgical technique or a standard procedure, and they will need to report how they feel about the results for at least six months after surgery.

Eligibility criteria
- Inclusion Criteria:
- • Male patients with deep system veno-occlusive erectile dysfunction
- • Lack of response to medical treatment for erectile dysfunction including PDEi (Phosphodiesterase Inhibitors), aphrodisiacs, hormonal therapy (if there is a hormonal disturbance), and intracavernous injections, despite control of risk factors such as diabetes mellitus.
- Exclusion Criteria:
- • Patients with uncontrolled diabetes, smokers and patients with anesthesia risk
- • Patients with arteriogenic erectile dysfunction, penile fibrosis, Peyronie's disease or psychiatric disorders.
- • Patients with bilateral deep system VOD
- • Patients who refuse to participate in the study
