This clinical trial is looking at a method called "deprescribing," which means reducing or stopping certain medications that may not be helpful or could be harmful for people living with dementia. The goal is to see if gentle reminders, or "nudges," can help both caregivers and healthcare providers make better decisions about medication use for patients with Alzheimer's disease, mild cognitive impairment, or other forms of dementia. The researchers want to find out if this approach can be easily used in real-world settings and if it can be measured effectively in future studies.

To participate in this trial, you must be at least 65 years old and have been prescribed a specific high-risk medication for a while. Your healthcare provider must be from Brigham and Women's Hospital and have prescribed one of these medications. If you qualify, you can expect to work with your healthcare team to assess your medications and potentially make changes that could improve your health and well-being. This study is currently recruiting participants, and it's an important step toward understanding how to better manage medications for those living with dementia.

Eligibility criteria

  • Primary care providers will be the primary unit of identification and randomization.
  • Primary care providers inclusion criteria:
  • Providers within Brigham and Women's Hospital primary care practices
  • Having prescribed at least one of the high-risk medications of interest
  • Eligible patients will be identified through this providers within the health care system.
  • Patient inclusion criteria:
  • At least 65 years of age
  • Prescribed at least 90 pills of one of the high-risk medications of interest in the last 180 days in the EHR system, which most guidelines consider chronic use, and have a diagnosis of dementia in the EHR.
