The Project CASCADE trial is focused on improving care for people who have survived cancer. The study aims to work with community health center doctors, nurses, and patients to create a better system for delivering care specifically tailored for cancer survivors. By testing this new approach, the researchers hope to see improvements in the health outcomes for patients and the experiences of their healthcare providers.
To participate in this study, you need to be between the ages of 65 and 74 and have visited a community health center at least once in the past year. You cannot have a history of cancer noted in your medical records. If you decide to join, you can expect to be part of a program that focuses on enhancing your healthcare experience as a cancer survivor. This trial is not yet recruiting participants, so it's a good idea to keep an eye out for updates if you're interested!

Eligibility criteria
- Inclusion Criteria (participants):
- • -at least one visit to the Community Health Centers (CHCs) in the prior year
- Exclusion Criteria (participants):
- • - no history of cancer recorded in the problem list or past medical history in the medical record
- Inclusion Criteria (clinicians):
- • -employed clinicians (e.g. physicians, nurses, pharmacists, or other allied health professionals) and non-clinician staff of the participating CHC clinics
