This clinical trial is studying two different methods of providing pain relief during labor: the Programmed Intermittent Epidural Bolus (PIEB) and the Continuous Epidural Infusion (CEI). The main goal is to see which method is safer and more effective for women in labor. Researchers will look at how each method affects movement, the amount of pain medication used, the length of time spent in the second stage of labor (when pushing begins), and overall satisfaction with pain relief.

To participate in the study, women must be over 18 years old and generally healthy, though some specific health conditions may exclude them. Once enrolled, participants will be randomly assigned to one of the two pain relief methods after providing their consent. Throughout labor, their vital signs and movement will be monitored, and after delivery, researchers will measure how much medication was used and how satisfied the women were with their pain relief experience. This study aims to help improve labor pain management for future mothers.

  • Inclusion Criteria:
  • age \> 18 years old
  • ASA 2 \& 3w
  • Exclusion Criteria:
  • patient refusal
  • significant comorbiddities, eg patient with heart disease, kidney diseae, neurological disoder
  • contraindicated to neuraxial blockade
  • allergic to local anesthetic agent or opioid
  • obstetric complications, eg pre-eclampsia, leaking liqour, suspicious CTG
  • morbid obesity, BMI \> 40
  • grandmultiparous ( \> 5biths at \> 20 weeks gestation)
  • combined spinal epidural technique for labour epidural analgesia
  • height \< 145cm
